Definitions of Material Death:-
• Maternal Death - Death of a woman while pregnant or within 42 days
of termination of pregnancy, irrespective of the duration and site of the
pregnancy, from any cause related to or aggravated by the pregnancy or
its management but not from accidental or incidental causes.
• Maternal Mortality Ratio (MMR) - Ratio of the number of maternal deaths
during a given time period per 100,000 live births during the same time-
period. The MMR is used as a measure of the quality of a health care
system.

: The MDSR system is a
continuous cycle of identification, notification, and review of maternal
deaths followed by actions to improve quality of care and prevent future
deaths.
• Confidential Review (CR): Multi‐disciplinary anonymous investigation
into all or a sample of maternal deaths, to critically observe the line of
management adopted in instances of maternal morbidities & mortalities
and to identify the avoidable or remediable factors associated with them,
so that the same could be corrected in the future

Neonatal Deaths: Neonatal deaths are deaths occurring during the neonatal period,
commencing at birth and ending 28 completed days after birth
Post-Neonatal Deaths: Deaths occurring from 29 days of life to under one year are
called post-neonatal deaths
Infant Deaths: Deaths of children less than 1 year of age
Child Deaths: Deaths of children less than 5 years of age
Still Birth: Still birth is the birth of a new born after 20th completed week of gestation,
commencing at birth and ending 28 completed days after birth
Post-Neonatal Deaths: Deaths occurring from 29 days of life to under one year are
called post-neonatal deaths
Infant Deaths: Deaths of children less than 1 year of age
Child Deaths: Deaths of children less than 5 years of age
Still Birth: Still birth is the birth of a new born after 20th completed week of gestation,
weighing 500gm or more, when the baby does not breath or show any sign of life after
delivery.
